Follow the instructions ; complete the form in its entirety
Print and bring FOUR copies of the form to registration.
*Volunteers need only TWO copies of their forms*

Please note entering your child into eayso does not register them, they are not registered until the Region has their forms and payment.

Registration Fee: $120.00 per child
Fee includes: Shorts, socks, jersey, pictures
Late Registration Fee (after July 5th): $125

AYSO Region 677 Refund Policy

A full refund of the registration fees will be given if the request is made prior to June 30.
July 1 to July 31 - A full refund minus a $20.00 administrative fee.
August 1 to August 30 -A partial refund of 50% of registration fees.
AFTER AUGUST 30th - NO REFUND if the player received a uniform or was placed on a team.

 

ALL refund requests must be made in writing using a REFUND REQUEST FORM (one per child), and mailed with a self-addressed stamped envelope to:
